Housing First is an alternative to a system of emergency shelter/transitional housing progressions. Rather than moving homeless individuals through different "levels" of housing, Housing First moves the homeless individual or household immediately from the streets or homeless shelters into their own apartments. Clients will likely present with complex and compounding health-related concerns. Typical demographics will have a serious mental illness that is often accompanied by substance abuse, history of involvement in the criminal justice system and chronic physical illness. A multi-disciplinary and team provides a service that is based on intensive case management and assertive outreach. ResponsibilitiesProvide in vivo follow-up support, assistance, and basic skill training to high-risk adult men, women and families in order to establish and maintain an independent and stable living the situation within the community that they have been recently housed in. Assist clients in a minimum 12-month time frame from intake to graduation with the goal of long term housing sustainability Broker client with existing community services including, identification acquisition, medical attention, legal resources, and income assistance. Implement harm reduction strategies. Clients do not have to be clean and sober to engage in the program. Work an intensive caseload of 12 clients per one-worker ratio (caseload size)Documenting all objective interactions with clients, whether by phone or face-to-face, on HIFIS. Ensure that all assessments and reports are completed in a timely manner before the 15th of every month Report directly to Team Leader and or Executive Director for clarification and major decision making Keeps track of all expense reporting including km tracking and direct client costs as well as client utility arrears and bills and hand in on a consistent basis Responsible for developing professional time management while working in the community With respect to client/landlord disagreement will support both client and landlord objectively with no bias toward either one Must have a personal motor vehicle and valid driver's license to transport clients to and from services in the community QualificationsDemonstrates knowledge/familiarity with related resources such as mental health systems, medical system and related agencies, income support system and related agencies and addiction support organizations. Willing to work mainly in the community and independently. Demonstrates suitability to work with disadvantaged and challenging adults in diverse environments. Demonstrates ability to communicate effectively both verbally and in writing. Demonstrates physical/mental ability to perform the duties of the job. Demonstrates ability to deal with others effectively Demonstrates ability to operate related It Equipment. Demonstrates ability to organize work and time management skills Demonstrates ability to understand and maintain client/worker boundaries. B.A./B.S.W./B.Ed or equivalent; or 3 - 5 years minimum of relevant experience. Enthusiasm for being part of the solution to end homelessness in Fredericton. Passion for working with marginalized individuals; excited about being a catalyst of change in people's lives. Must have the ability to empathize with those experiencing significant barriers in their lives. A significant understanding of the issues of poverty is essential. Understanding of and passion for housing first programs and policies. Able to show a demonstrated commitment to the agency's mission and values, policies and guidelines.
