Job Description This posting is in collaboration with Bloordale Pharmacy. The Nurse Practitioner will be working for Bloordale Pharmacy. *Summary*: The *Nurse Practitioner* will work in conjunction with the family physician to ensure an effective patient care process. *The Nurse Practitioner will work 5 days a week, with supervision by one physician. The pay range is $50-60 per hour. * *Location*: 214 Main St. Hawkesbury, ON *Responsibilities*: * Provide primary care to patients in-person * Coordinate care with in-house physicians and pharmacist team * Perform proper documentation and record keeping to fulfill the medico-legal responsibilities * Understand and be compliant with clinic process and procedures * Activities including and not limited to - conducting patient interviews, histories, arranging follow up care appointments * Manual charting (no EMR). *Requirements*: * Active practice registration with the College of Nurses of Ontario * Masters of Nursing - Nurse Practitioner or equivalent educational preparation * Advanced nursing care knowledge related to health assessment, diagnostic, pharmacotherapeutics and invasive and minor surgical procedures required * Minimum of one (1) year recent primary care nursing experience required * Excellent patient focus and communication skills * Bilingual (French/English) is preferred Job Type: Permanent Schedule: * Monday to Friday * Weekends as needed Application question(s): * Do you have a Masters of Nursing - Nurse Practitioner or equivalent degree?
