Who You AreExperienced registered nurseDemonstrated ability to work within a multidisciplinary teamStrong interpersonal communication skillsExtensive knowledge in nursing role and enhances scopeWhat You?ll DoIndividual diabetes education sessions with patientsTeach patients how to monitor blood glucose and educate on blood glucose targetsEducate patients on medication management and insulin regimensProvide guidance on nutrition and physical activityEducate patients and families on diabetes prevention, self-management and complicationsParticipate in community outreach and education initiativesWillingness to travel to coastal sites on a travel schedule What You BringDiploma in Nursing, BScN preferredCurrent registration and in good standing with College of Nurses of OntarioMandatory Certified Diabetes Educator certification or willingness to obtain within 2 years of hireMinimum 2 years? experience working in diabetes education or clinical care/chronic disease preventionMandatory certification in CPRReliable, cooperative and effective interpersonal relationshipsAbility to communicate in the local native Cr- Location Moose Factory, ON
