Summary The Elders Wellness Coordinator is responsible for supporting the wholistic well-being of Elders across communities, with a focus on dementia care, physical wellness, social connection, and cultural engagement. Reporting to the Traditional Wellness Manager and working collaboratively with the Traditional Wellness Team, the Elders Wellness Coordinator delivers ongoing wellness programming, outreach services, and home visits for Elders who are unable to attend on-site programming. This role ensures services are culturally relevant, accessible, and responsive to the evolving needs of Elders, while fostering strong relationships with communities, caregivers, and service providers. Job DutiesDevelop, coordinate, and deliver ongoing wellness programming that supports Elders? cognitive, emotional, physical, and social well-being.Facilitate group activities such as sharing circles, wellness education sessions, light recreation, and social gatherings.Incorporate culturally relevant and land-based activities, working alongside Elders, Knowledge Keepers, and cultural practitioners where appropriate.Conduct home visits for Elders who are unable to attend programming on-site, providing one-on-one support, wellness check-ins, and companionship.Support caregivers and families by providing guidance, education, and connection to available services and resources.Promote social connection and reduce isolation through meaningful engagement and relationship-building.Provide general nutritional guidance and support healthy lifestyle practices for Elders.Lead safe, accessible movement and fitness activities tailored to Elders? abilities.Conduct basic mobility, balance, and wellness assessments to support individualized and group programming.Modify programming to ensure inclusivity, accessibility, and safety for all participants.Coordinate programming schedules, materials, and space requirements in collaboration with Elders Centres and community partners.Build and maintain respectful, collaborative relationships with First Nations communities and local service providers.Participate in community events, gatherings, and outreach initiatives to promote Elders wellness services.Identify emerging needs or concerns and connect Elders to appropriate internal or external supports.Maintain accurate records of participation, activities, and outcomes, and contribute to reporting requirements.Gather feedback from Elders and community partners to support program evaluation and continuous improvement.Travel regularly between communities and sites to deliver programming and outreach services.Ensure program spaces are set up, maintained, and restored following activities.Maintain inventory of supplies and coordinate the purchase of materials as required.Adhere to all organizational policies, procedures, confidentiality requirements, and health and safety standards.Complete required training, including the Seniors Fitness Instructor Program (if not already certified).Work collaboratively with internal teams to support integrated, wholistic care for Elders.Other duties as assigned.Requirements· Diploma in a health or social service related fieldExperience working with Elders, seniors, or individuals living with dementia.Valid Class G Driver?s License with reliable access to a vehicle for travel between sites.Current CPR-C and First Aid Certificate (or willingness to obtain).Clear Police Vulnerable Sector Check.SkillsKnowledge of health and wellness principles, including nutrition, social engagement, and movement-based activities.Understanding of, and respect for, Anishinabek culture, traditions, and approaches to wholistic wellness.Strong interpersonal and relationship-building skills.Effective verbal and written communication skills.Organizational and time management skills to coordinate programming across multiple sites.Ability to document activities and maintain accurate records.AbilityAbility to work independently with minimal supervision while cont- Location Little Current, ON
