FHC is a registered charity and an incorporated not-for-profit Community Health Centre (CHC), with a vision of Strong Healthy Communities. Across our three sites, we provide a range of health-related services based on the social determinants of health with extensive community engagement.</p><p><br> At FHC, we believe that health is much more than just the absence of disease. Our approach to community health encompasses the social determinants of health which includes: education, employment, isolation, food security and social supports, and utilizes a community development model to promote health, prevent disease, and strengthen community capacity. We value health equity, inclusion, community engagement, accountability & transparency, excellence and collaboration & partnerships.</p>Position Summary:<p>The successful candidate will be a part of the Chronic Disease and Mental Health multidisciplinary team, connecting with and engaging community partners through environmental scanning and assessments, conducting extensive outreach, coordinating and supporting various community chronic disease-related programs and services as well as taking a lead in program evaluation and quality improvement initiatives.</p><p></p>Responsibilities include:<ul><li>Liaise and network with community agencies, leaders, and members around chronic disease prevention and management</li><li>Outreach to various healthcare providers, internal and external</li><li>Promote diabetes education and other chronic disease-related program, including tele-ophthalmology - in person, on social media, FHC?s website, in community events and fairs, to encourage referrals, recruit program participants and create general community awareness about FHC and the services we provide</li><li>Support the delivery of chronic disease education sessions on/offsite</li><li>Coordinate the full team?s chart audit, program evaluation and quality improvement initiatives</li><li>Gather and organize and analyze data (including health equity), maintain accurate records, complete and submit regular progress and evaluation reports</li><li>Address queries and answer phone calls and correspondence pertinent to the Chronic Disease services and programs</li><li>Support the tracking of emergency food cards, transportation support and interpretation services across the full team</li><li>Complete all administrative duties associated with this role</li><li>Cross-collaborate with colleagues and providers within the organization</li><li>Assist in the recruitment, training, supervision and engagement of volunteers</li><li>Participate in internal/ external committees, as required</li><li>Promote and support attainment of organizational- Location North York, ON
