CASE MANAGEMENT COORDINATORJOB DESCRIPTIONOVERVIEWThe Case Management Coordinator (CMC) plays a key role in leading the coordination and delivery of trauma-informed, feminist, client-centered support to women, children and families accessing Liberty Lane's second-stage housing and outreach services.Reporting to the Program Director and working in close collaboration with the Domestic Violence Project Coordinator, the CMC leads the delivery of services and programming within Liberty Lane's Family Stream, including children's programming and services for individuals within residential and outreach programs.The CMC is responsible for a small individual caseload (approx. 6 clients per month) while also providing case management consultation to other members of Liberty Lane's second stage and outreach service delivery team.In addition, the CMC works collaboratively with the Domestic Violence Project Coordinator to supervise the Family Case Workers, Child and Youth Workers, volunteers and students who compose Liberty lane's second stage and outreach service delivery team.KEY RESPONSIBILITIESCoordination and SupervisionCollaborate with the Program Director and Domestic Violence Project Coordinator to provide general supervision and support to members of Liberty Lane's second stage and outreach service delivery teamServe as the direct supervisor for up to 4 Case Workers, includingholding monthly 1:1 meetings with assigned staff to monitor workplans, well-being, caseloads and provide assistance in navigating any other general supports required by Case Workersmaintaining meeting minutes and related documentation to support performance managementproviding general input and guidance on case management processes and client work in collaboration with the Domestic Violence Project Coordinator, Program Director, and other external supports as neededSupport the Program Director in planning and facilitating monthly Program meetings, including maintaining up-to-date, accurate meeting records and following up on action itemsIdentify and highlight opportunities, challenges, or issues of concern related to case management and/or programming to the Program DirectorCase Management and Client SupportCollaborate with the Program Director and Domestic Violence Project Coordinator to maintain and monitor client inquiries for services, waitlists, and case assignmentsConduct intake, assessment and individualized case planning for Liberty Lane's Family StreamAttend monthly Coordinated Community Response meetings as neededProvide one-to-one crisis and long-term support for residential and outreach clients and their children within the Family Stream (approx. 6 clients), including:supporting adult clients in goal setting related to safety, housing, parenting, employment, and wellnesssupporting parents with goal setting related to safety planning, housing, parenting challenges, employment and general wellnesssupporting the children of survivors to enhance emotional regulation, social and coping skills within a child centered, play-based frameworkfacilitating referrals to community resources, including counselling, education, and legal supportassisting clients with applications for housing, financial assistance and other supportscollaborating with schools, child welfare agencies and other community resourcesAdministration and Other DutiesMaintain accurate and confidential records, case notes, statistics, and other necessary documentation in accordance with Liberty Lane's requirementsPrepare regular reports for the Program Director, Executive Director, Board of Directors and others as neededParticipate in staff meetings, supervision, training, and professional developmentAssist the Program Director with human resources management, strategic development and other initiatives as neededREQUIREMENTS:Ability to work in-person at Liberty Lane's Fredericton officePost-secondary degree in Social Work, Psychology, Counselling, or a related field, or equivalent combination- Location Fredericton, NB
